Strategic Consulting

We help health providers and systems navigate rapidly changing reimbursement and compliance environments. Karna’s senior health professionals can inform evidence-based programs and advise on specific pragmatic strategies. By applying what is already proven to novel circumstances, we arrive at program designs that innovate and create greater patient and staff engagement. Our solutions establish a repeatable strategic framework from which to launch and sustain multiple programs.

Provider Outreach & Training

As the industry shifts towards value-based payment models, providers must find and implement new ways to create and deliver value for their patients and payors.  We believe that outreach and patient engagement creates this value through improved outcomes and reduced costs. Karna designed our programs to transition care away from reactive, interventional care to more proactive population health. Our programs use available data to pinpoint appropriate evidence-based interactions for the most at-risk populations.  Experienced Karna clinicians (physicians, advanced practitioners) train clinical and support staff on the outreach program so that message context is understood and communicated effectively.  We focus on helping with culturally and linguistically appropriate communication strategies, prioritizing the medical conditions and common scenarios that drive the most cost and result in the best clinical outcomes.  In addition to patient engagement and outreach programs, we deliver Continuing Medical Education/Continuing Education programs that are scientifically defensible, clinically relevant, and drive outcomes that are reproducible, all while also allowing for necessary credits for licensure.   We deliver outreach and training across all media, live, on-line, and self-driven.  We measure results, use what works, and discard what will not.

Telehealth / Rural Access Integration

Access to medical providers in rural areas is limited and as a direct result, clinical and cost outcomes in these underserved areas are predictably below average.   Karna’s strategy to increase access to appropriate care in rural areas emphasizes technology to extend the reach of the best medical providers into areas that are difficult to reach routinely.  We engage health systems to extend care to rural areas for primary care, Opioid Use Disorder treatment, and occupational injury care.  For example, we perform evaluation and design for systems that want to engage patients remoted via telemedicine for physicians or counselors.  We provide the logistics, technology, and clinical training necessary to make this transition possible.

Prevention Strategies

Where health systems and/or practices lack access to certified educators (e.g. diabetes prevention, healthy eating, etc), Karna delivers validated prevention strategies and services to address their patients’ chronic disease management needs.  We ensure that telehealth is part of a thoughtful system of care, not the endpoint in a complex continuum.  Our integration process includes continual evaluation and revisions – we aid in adoption of what works for clinicians, operations professionals, and patients, careful to understand current compliance and reimbursement implications.

Remote Patient Monitoring

Providers continue to improve their ability to remotely monitor patient measurements, activities, and outcomes to increase access to expert care and reduce delivery costs. It is widely accepted that remote patient monitoring results in faster identification and tracking of patients most at risk of serious complications, often resulting in hospitalization.  We help integrate strategies for primary care, specialty, and multi-specialty systems.  We integrate the patient experience with clinic operations to yield a high-adoption, high-fidelity, cost-effective, clinically relevant monitoring solution that reduces administrative burden and increases likelihood of patient retention and satisfaction.  We measure both clinical and financial outcomes to make entry, persistence and scalability of a monitoring program a series of logical steps that are backed by evidence.

Occupational Medicine

Karna supports both the art and science of commercial occupational medicine by providing strategic, medical, operational and technical consulting to providers of occupational medical treatment services.  We integrate full lifecycle occupational medicine services across the continuum to include designing appropriate preemployment testing through drug screening, ergonomic testing, injury prevention and treatment, ancillary services integration, telehealth and remote patient monitoring integration and on-site and mobile services). Karna offers particular expertise in training primary care and urgent care medical providers on the nuances of effectively managing occupational injuries in a manner that is O.S.H.A. compliant and seeks to minimize costs and recovery times.

Opioid Use Treatment Integration and Coordination of Care

Karna designs programs to coordinate care, provide community education, psychosocial services, interventions and recovery support for long term improvement in opioid use disorder treatment.  Our treatment integration framework allows for more effective provider collaboration in coordinating care among patients undergoing treatment.  We provide assistance calculating Return-On-Investment (ROI) valuation of existing programs and estimate cost savings in proposed systemic treatment strategies to ensure the most sustainable programs.  We provide technical, clinical, and logistics training and support.

Discharge Management

Given the shift to value-based reimbursement, we work with health systems to improve the measurement, logistics, and improvement of hand-offs necessary to effectively discharge patients into appropriate follow-up care. We measure ROI and cost-avoidance around inappropriate discharges, and especially in poorly coordinated clinical hand-offs.  We integrate remote patient monitoring, telehealth, and community resources in order to optimize discharge management strategies.

Outcomes Analytics

Karna develops qualitative and quantitative evaluation frameworks, strategies, and reproducible outcomes analyses for health systems. Our approach utilizes scientifically defensible methods to perform impact analyses, ROI calculations, and strategy projections for health providers.  We combine modern data architecture with business informatics to effectively design and validate current outcomes using clinical, administrative, or research data. We deliver clinical context to administrative data so that decision support strategies can be appropriately developed and interpreted.
